A document of approval from the President of the Republic of Belarus, head of a state authority or other state organization about completion of thesis (for persons specified in paragraph 39 of the Statement about conferment of academic degrees and conferral in the Republic of Belarus, approved by Order of the President of the Republic of Belarus of the 17th of November, 2004 № 560 (National register of legal acts of the Republic of Belarus, 2004, № 180, 1/6013; 2011 г., № 137, 1/13123).

4. Autobiography *.
5. A copy of Education Certificate (Diploma) of Higher Education with Final official transcript or a copy of any other document of higher education which is equivalent to the Ist stage of higher education in the Republic of Belarus with a list of studied disciplines and marks (grades).
6. A copy of Master's Degree Diploma and a Qualification Record from examination-and-test register or a copy of any other document of higher education which is equivalent to the IInd stage of higher education in the Republic of Belarus with a list of studied disciplines and marks (grades) (for citizens of the Republic of Belarus – if available).
7. A copy of Internship Certificate (for citizens, admitted to PhD course on a group of specialties «Clinical medicine»).
8. A copy of Certificate about passing candidate’s tests (differential pass-fail exams) and candidate’s exams on general disciplines (for persons, who apply to PhD course for getting scientific qualification «Researcher» extramurally or intramurally).
9. Recommendation letter from a scientific council or faculty of higher education establishment (for persons, who apply to PhD course for getting scientific qualification «Researcher» on the year of getting higher education at educational establishment) or a copy of work record book or any other documents, which prove work experience (for persons, who apply to PhD course to get the qualification «Researcher»).
Persons with higher education in the field «Healthcare», awarded by a qualification «Physician», while applying to PhD course at educational establishments and organizations of the Ministry of Healthcare of the Republic of Belarus, which implement educational programs of postgraduate education, on a group of specialties «Clinical medicine» without doing internship course and (or) without 2-year work practice, submit a recommendation approved by Higher Attestation Commission of the Republic of Belarus and Ministry of Health of the Republic of Belarus.
10. Application of the organization – staff customer (if applicable).
11. A list and copies of publish research papers, in the absence thereof – a scientific report on a chosen specialty (for persons, who apply for PhD course to get a scientific qualification «Researcher»).
12. Documents, which prove participation in scientific-and-research and innovative projects, copes of materials and (or) theses of reports from scientific, scientific-and-practical conferences, meeting, symposia and other similar events (if applicable).
13. Conclusion of a scientific seminar of a laboratory (meeting of a department) about rationale, quality and dimensions of individually completed researches on the topic of the thesis being prepared (for persons, who apply to PhD course to get the scientific qualification «Researcher» in the form of joint research).
14. Documents which give preference for admission to PhD course to get a scientific qualification «Researcher» (if applicable).
15. Three photos with the size 4 x 6 sm (for persons, who apply for PhD course to get a scientific qualification «Researcher» in the form of joint research, and persons, who apply for PhD course to undergo training in the form of joint research aiming at passing candidate’s exams and credits (differential pass-fail exams) on general disciplines, – 2 photos).
16. Statement of good health (for citizens of the Republic of Belarus).
17. Medical Consultation Commission Certificate, given by a territory healthcare organization of the Republic of Belarus (after a compulsory medical examination by the assignment of educational establishment, organization, which realizes educational programs of postgraduate education) for foreign citizens and person without citizenship.
18. Medical assessment about health condition and HIV-negative certificate, given by an official healthcare organization of the country of an applicant who applies to PhD course for foreign citizens and persons without citizenship.
19. A copy of a passport and equivalent document, a document which proves the right of residence in the Republic of Belarus, a contract of compulsory health insurance for foreign citizens and persons without citizenship.
Foreign citizens and persons without citizenship, who are applying to PhD course to get a scientific qualification «Researcher» or to study in the form of joint research aiming at passing Candidate’s exams and Candidate’s credits (differential pass-fail exams) on general disciplines in accordance with international contracts of the Republic of Belarus, contracts on completion of international programs, contracts of educational establishments, organizations of the Republic of Belarus, which have educational programs of postgraduate education, with organizations of foreign countries (international organizations) or by assignment of an organization of a foreign country, additionally submit a copy of one contract mentioned above or assignment of an organization of foreign country .
In addition to the documents in a foreign language, mentioned above, it is obligatory to have their notarized translation copies in the Russian or Belarusian languages. A document of identification and original documents about education are submitted by a PhD applicant personally.
*It has to be recorded in accordance with the rules of the Instruction on the order of formation, maintaining and storage of personnel records, approved by a decision of a Committee on archives and records management of the Council of Ministers of the Republic of Belarus of the 26th of March, 2004 № 2 (national register of regulatory acts of the Republic of Belarus, 2004 , № 58, 8/10795).
